Who doesn’t have a sense of déjà vu about further restructuring of the NHS? Integrated care systems (ICSs), integrated care boards (ICBs), place based care, neighbourhood based care—more labels and acronyms to learn, while trying to understand it all.As with any changes in the NHS, there are many perspectives on what they mean. Some people believe that creating such bodies opens the door to greater commercial involvement and takes the NHS closer to privatisation. At the other extreme are those who approach it with zeal, as a lifetime opportunity for much needed reform and structural reorganisation.Between those two extremes (and I’m not knocking either of them) sit most people working in the NHS. Most remain oblivious, apart from a flurry of emails from higher up “explaining” the changes. The biggest crisis we have is around workforce, and we won’t solve that by rearranging structures.
