I agree that we need a new public conversation about how to improve the NHS.1 It will be challenging, however, to engage in serious discussion.We need clarity on what to discuss and how. Simply asking the public about the founding principles will elicit “strong support”—but does the public know the reality of the current NHS? Are they aware that it is a bottomless pit depriving other essential services such as social care, housing, policing, education, and others of resources, and therefore is itself becoming a cause of inequalities? Do they know that the NHS machinery is wasteful, the quality of its services is questionable, and best practices are slow to be adopted?We also need to talk about various expert reports, including the recent BMJ Commission, which either hark back to a golden age or project a utopian future.
