Patients are “constantly being inconvenienced, endangered,” or denied opportunities to improve their long term health because of a lack of joined-up care in the NHS, a report by the House of Lords has warned.1The Lords’ committee on integration of primary and community care interviewed academic experts, government officials, former ministers, and representatives from healthcare, charities, and patient groups as part of a review into how primary and community services could be integrated to provide holistic care.The report found that a lack of coordination between primary care and community services was leading to “substandard care, missed opportunities for home or community based treatments, and undue strain on hospitals that are already overstretched.”It highlighted evidence provided by the British Red Cross, a representative of which said, “[People] tell us that they have almost reached that point of despair where they feel as though they are not able to access the services they…
